The granulomas are soft because they contain necrotic centres. Macroscopically we can see caseous necrosis. The granulomas are more numerous in the upper lobes. This is because the upper lobes are better ventilated (see pathophysiology), and the ''m. tuberculosis'' bacterium is highly aerobic.
